In your shoes at this point, given all the changes that have been made, I'd delete the data array in Megaraid storage manager and recreate it. Set all the settings you have there, and use a 64K strip.
When you format the disk in Windows, use 64K there as well. Do a fast initialization at the end of the creation of the logical volume in MSM so it presents it to Windows right away. Even during LSI 9266-8i MegaRAID SAS initialization it should perform better than what you're seeing.
Do not make any changes at all to the disk in windows. If this doesn't help, I've gotta think bad firmware.
